Auto responder filling in wrong info

Hello,

I just set up an auto responder for a free download. When someone downloads the e-book by filling out a form they are added to my email list and I get a notification. Unfortunately the notification is filling in the 1st person who filled it outs name and info every single time. What's going on?

Any help would be great.
