Custom form not emailing notifications

I have set up a simple form on our site. Everything works the way it is supposed to except no confirmation/notification emails are sent out when the form is submitted. I've looked everywhere for a reason but have come up empty handed. I've watched the videos, checked the page types, etc. but it just won't send any email. I have also checked spam filters, etc in case it was being caught up in there and still a no go. Anyone know why this would be happening? (The email is being sent to a gmail account as a test.)
